The challenge
Field teams used pool vehicles for clinic and outreach runs. Supervisors received end-of-day paper sheets. Donor audits asked for evidence that assigned villages were served. GPS existed on paper contract but was not tied to trip completion.
What went wrong
- Visit form timestamp did not match vehicle arrival geofence
- Same driver “completed” two distant sites within impossible window
- Vehicle parked at home location during declared field hours
